雍正元年正月日　-　January 1st, 1st Year of the Year of Ongjeong (Yongzheng) (1723)
(Adopted era name of the Qing Dynasty)

尚瑞院印　-　Seal of the Secretariat for State Affairs

This is an official horse warrant for one saddle horse, used for the use of official horses during the Korean Joseon Dynasty. There were warrants for one horse to five horses.

thank you so much for information .there are 2 medals 1 with 5 horses that looks a little different

Only the 2nd line from right is different. The rest is same.
The different line says,
宇字號五馬牌　Number Heaven, Five Horses Tag
