Subject:Japanese figure - Chinese(?) translation

Dear All,

Please, may i have some help in translating an inscription on the bottom of my Japanese wood carving?

The figure is one of the Four Guardian Kings. He's 98cm tall, carved in sections and joined. I've always supposed him to be from the Kamakura dynasty; or perhaps a bit later.

Recently, i took him to an auction house to appraise - not that they had much idea what he was. But, they did notice an inscription to the bottom of the base 'plank'.

I am hoping it gives some detail as to which temple the figure guarded, and some date.

泉北大鳥郡上神谷 鉢峯山 
Izumi Kita Ōtori-gun Niwadani Hachiminesan (Temple) - Now a part of Sakai city, Osaka prefecture、Name of temple - Hōdōji
十二神将 - Twelve Heavenly Generals
十二躰之内 - One of Twelve
